TEAMWORKS requires all participants to have a TEAMWORKS Membership Card prior to participating in any program.  These cards are good for one year from the date of purchase & can be acquired at any time in the TEAMWORKS office.  These cards also provide members with special discounts.

• Youth Membership:  $10 

Please have the following participant form filled out when purchasing a membership.

To avoid the rush, we recommend purchasing membership cards at least one week prior to the start of the program.
